We connect high-quality, mobile GPs with aged care homes to ensure residents receive consistent, patient-centred medical care.

Our mission is to become the go-to partner for RACFs across Australia, providing a coordinated, sustainable, and high-trust GP service model that improves care outcomes and reduces the administrative burden for facilities.

How We Partner with RACFs

GPAC-N works alongside facility managers, clinical leads, and nursing teams to:

  • Coordinate regular GP visit schedules

  • Ensure clear lines of communication between GPs and RACF staff

  • Support shared care plans and multidisciplinary case conferences

We act as both the patient’s clinical support partner and want to add value to your team, ensuring your facility receives both responsive care and long-term planning continuity.

  2. We conduct a short RACF needs assessment with you

  3. We match you with a dedicated GP or GP team

  4. We onboard facility staff to communication and documentation processes with our systems.

  5. Residents are registered with their GPs via MyMedicare, and our system.

We also provide family-facing materials to help residents and their families understand the model and what to expect.
