How We Partner with RACFs
GPAC-N works alongside facility managers, clinical leads, and nursing teams to:
Coordinate regular GP visit schedules
Ensure clear lines of communication between GPs and RACF staff
Support shared care plans and multidisciplinary case conferences
We act as both the patient’s clinical support partner and want to add value to your team, ensuring your facility receives both responsive care and long-term planning continuity.
