Collaborative Aged Care Medicine

GPAC-N supports a multidisciplinary approach to residential aged care. Alongside GPs, we work with allied health professionals and medical specialists who provide expertise in areas commonly required in aged care medicine.

These clinicians play an important role in supporting complex care planning, medication reviews, mental health management, rehabilitation, and preventative care.

Our network provides opportunities for allied health professionals and specialists to collaborate with GPs caring for residents across supported RACFs.

Allied Health & Specialist Opportunities

GPAC-N works with a network of allied health clinicians and medical specialists who support the care of residents in Residential Aged Care Facilities. These roles help ensure residents receive comprehensive, coordinated care alongside their GP.

Geriatricians

Support GPs managing complex geriatric conditions including frailty, dementia, polypharmacy, and functional decline. Collaboration may include case discussion, clinical advice, and specialist consultation where required.

Psychiatrists

Provide specialist input for behavioural and psychological symptoms of dementia, mood disorders, and complex mental health presentations common in aged care settings.

Pharmacists

Participate in Residential Medication Management Reviews (RMMRs), medication optimisation, deprescribing initiatives, and collaborative medication safety programs with GPs.

Allied Health Professionals

Physiotherapists, dietitians, occupational therapists, psychologists, and other allied health clinicians contribute to rehabilitation, mobility, nutrition, and mental wellbeing of residents.

Integrated Care Model

All clinicians participating in the GPAC-N network work alongside GPs and facility nursing teams to support coordinated, resident-centred care within RACFs.

Expressions of Interest

GPAC-N is building a network of allied health professionals and specialists interested in contributing to aged care medicine. We welcome expressions of interest from clinicians who would like to collaborate with our GP network.
