Our plan
Strengthen onboarding for new facilities and patients so relevant information is available before consultations.
- Use the Nurse Practitioner or clinical team to review and update records and health summaries where appropriate.
- Work with facilities to improve access to their clinical systems.
- Make medication information and recent clinical updates readily available to the treating GP.

After-Hours Care
After-hours arrangements may vary by facility and by provider. Residents and families should first contact the Residential Aged Care Facility for urgent clinical concerns.
Where supported, after-hours care may be provided through the resident’s GP, the facility’s usual after-hours pathway, or an external after-hours service.

Need help after hours?
Speak with the Residential Aged Care Facility first so they can arrange the appropriate care pathway.
Where local GP or facility pathways are not available, 13SICK may be used as an alternative back-up after-hours option, subject to availability and eligibility.
